Job Description:

Registered Nurse (RN)/Licensed Practical Nurse (LPN)  will provide medical support, provides quality care to patients and performs technical skilled care in patient’s home in compliance with nursing standards, federal, state, local government regulations and company clinical policies.

Responsibilities:

·       Provides direct and individualized nursing care to assigned patients based on nursing standards and under the supervision of the Director of Nursing and/or a Charge Nurse designee.

·       Ensures quality and safe delivery of clinical services to patients.

·       Provides accurate and timely documentation consistent with plan of care.

·       Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards.

·       Assists patient in learning appropriate self-care activities.

·       Uses equipment and supplies effectively and efficiently.

·       Documents and reports activity as required.

·       Will perform other reasonable related duties as assigned by management.

Requirements:

·       Active Licensed Practical Nurse in state of practice. RN license in state of practice 

·       Graduate of an accredited practical nurse program.

·       BLS Certification issued through American Heart Association required.
