Job Description:

This Clinical Supervisor position will be expected to proactively contact, schedule and complete Medicare Annual Wellness visits for qualifying patients in the clinic setting. Will work independently, but collaboratively, with the providers and clinical teams at multiple clinic locations to complete patient clinical visits in an efficient manner, follow protocols and standing orders including completion of preventative services and complete appropriate EHR documentation after patient visits.

Requirements:

• Active Licensed Registered Nurse (RN) in state of practice. 
• Graduate of an accredited school of nursing. 
• 2 years of nursing experience REQUIRED.
• CPR certification with American Heart Association. 
• Able to assess, plan, implement and evaluate nursing care.
• Supervisory Experience Required!
