Job Description:

We are looking for a Community Liaison assist in the transition from Nurse Facilities to Community living. Liaison will be responsible for nurturing and developing relationships with their clients/family. This role requires strong interpersonal skills and the ability to effectively communicate with a diverse range of individuals. The ideal candidate will have a passion for community engagement and be dedicated to improving the lives of those in the community.

Responsibilities:

  • Assist in the transition from facility to home with proper care teams.
  • Point of contact for the family, healthcare proxy, and nursing facilities.
  • Finalizes referrals to other programs and follow-ups.

Requirements:

  • High School Diploma or equivalent with 1 year of related work experience
  • Interest in working in nursing facilities
  • Ability to make proper judgments, assessments, superior written and verbal skills, and ability to maintain effective interpersonal relationships with staff members, consumers, and partners.
  • Proficient in the use of computer systems, including but not limited to, Windows based software applications such as Word, Excel and Outlook. 
  • Ability to process information accurately and handle confidential materials with discretion.
  • Excellent interpersonal and communication skills.
  • Strong organizational and project management skills.
  • Ability to work independently and as part of a team.
  • Passion for community engagement and improving the lives of others.
  • Knowledge of community resources and trends.
  • Must have a valid driver’s license and reliable transportation to Nursing facilities
