Job Description:

The Clinical Care Manager provides high-quality clinical consultation, supervision, and support to the Care Teams. Through the coordination and integration of care, the Clinical Care Manager works to address the behavioral health and/or needs of the individuals served by providing crisis management support, and care coordination working with the Care Teams to reduce member utilization. The Clinical Care Manager oversees the timely completion of high quality comprehensive health assessments and person centered care plans to ensure compliance with programmatic requirements.

Requirements:

• Licensure at independent practice level (LCSW, LICSW, LMHC, LMFT, LADC I, RN with BH expertise, PhD, OT, licensed rehabilitation counselor)
• Minimum of two years of relevant professional experience strongly required
• Bachelor's or Master's degree in Social Work.
• One year of experience as a Social Worker in a medical setting.
• CPR Certification. 
• Ability to perform the essential functions of this position, with or without reasonable accommodation and without posing a direct threat to the health and safety of self or others. 
• Ability to comply with all hospital policies and procedures that are job-related and consistent with business necessity. 
• Ability to effectively communicate with patients and provide competent care as appropriate to the ages of patients.
• Computer literacy is desired.
