JOB TYPE: Full-Time Permanent


Job Summary:


Under the general supervision of the Director of Pharmacy Services the Clinical Pharmacist will assume the responsibility

and accountability for the management of rational antibiotic therapy, or management of

the critical care or high risk patients as defined by area of responsibility. Will provide for the collaboration and consultation

with other health professionals on the therapeutic use of antibiotics or other medication for patients under the scope of

responsibility. Will also provide education and training for health professionals involved in this clinical forum



Responsibilities:

  • Provide clinical pharmacy services, including medication therapy management, drug information, and patient education, to patients and healthcare providers.
  • Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy for patients.
  • Review and interpret patient medical histories, laboratory results, and other relevant information to determine appropriate medication therapy.
  • Monitor and assess patient medication therapy for effectiveness and adverse effects, making recommendations as necessary to healthcare providers.
  • Participate in medication safety initiatives, including identifying and reporting adverse drug events and medication errors.
  • Provide education and training to healthcare providers and patients on medication-related topics.
  • Develop and implement protocols and guidelines for medication use in specific patient populations or disease states.
  • Serve as a drug information resource to healthcare providers, patients, and caregivers.
  • Participate in formulary management, drug utilization review, and other pharmacy-related activities.


  • Participate in research and quality improvement initiatives related to medication therapy.

Requirements:

  • Graduate from an accredited school of Pharmacy with a PharmD degree.
  • Current New York State Registration.
