A Home Health Registered Nurse Case Manager is a licensed registered nurse who works with patients in their homes to provide medical care and help them manage their health. The main responsibility of a Home Health RN Case Manager is to coordinate and manage the patient's care plan and treatment, working with the patient, their family, and other healthcare professionals.



The duties of a Home Health RN Case Manager may include:

  1. Conducting patient assessments: This involves assessing the patient's physical, emotional, and mental health to develop a comprehensive care plan.
  2. Developing care plans: Based on the patient's needs, the RN Case Manager will develop a personalized care plan that includes the patient's goals, treatment plan, and medication management.
  3. Coordinating patient care: The RN Case Manager will coordinate with other healthcare professionals, such as physicians, therapists, and social workers, to ensure the patient receives comprehensive care.
  4. Providing direct patient care: The RN Case Manager will provide direct patient care, including wound care, medication administration, and other treatments.
  5. Educating patients and their families: The RN Case Manager will educate the patient and their family on the patient's condition, treatment plan, and other important information.
  6. Documenting patient progress: The RN Case Manager will document the patient's progress in their medical record and report any changes in the patient's condition to other healthcare professionals.
  7. Adhering to regulatory standards: The RN Case Manager must adhere to regulatory standards and guidelines, including those set by the state and federal government, and the healthcare facility.


Qualifications:

  1. Must have a valid nursing license in the state of California.
  2. Should have at least two years of clinical nursing experience, required in a home health setting.
  3. Possess excellent communication and interpersonal skills.
  4. Should have knowledge of medical terminology, medication management, and basic nursing skills.
  5. Should have experience working with patients with chronic illnesses and disabilities.
  6. Must be willing to work independently and as part of a team.
  7. Must have a valid driver's license and reliable transportation.
