Job Summary:

As a Physical Therapy Assistant at our Skilled Nursing Facility, you will play a crucial role in providing high-quality rehabilitation services to our residents. Under the supervision of a licensed Physical Therapist, you will assist in the development and implementation of individualized treatment plans to help residents regain mobility, strength, and independence. Your contributions will have a significant impact on the well-being and recovery of our residents.


Key Responsibilities:

  1. Patient Assessment: Conduct initial and ongoing assessments of residents to determine their physical and functional status.
  2. Treatment Planning: Collaborate with the supervising Physical Therapist to develop individualized treatment plans based on resident assessments and goals.
  3. Therapeutic Interventions: Administer therapeutic exercises, modalities, and manual techniques as prescribed by the Physical Therapist to improve residents' physical function and mobility.
  4. Documentation: Maintain accurate and detailed records of resident evaluations, treatment interventions, and progress notes in compliance with facility policies and regulatory requirements.
  5. Education: Provide education to residents and their families regarding the goals, benefits, and techniques of physical therapy treatments.
  6. Collaboration: Work closely with the interdisciplinary healthcare team to ensure coordinated and comprehensive care for residents.
  7. Safety: Adhere to infection control protocols, safety measures, and ethical standards to maintain a safe and supportive rehabilitation environment.
  8. Communication: Communicate effectively with the Physical Therapist, nursing staff, and other team members to provide updates on resident progress and adjust treatment plans as needed.


Qualifications:

  • Associate's degree from an accredited Physical Therapy Assistant program
  • CA state licensure as a Physical Therapy Assistant
  • BLS (Basic Life Support) certification
  • Strong interpersonal and communication skills
  • Compassion and empathy for the elderly and individuals with various medical conditions
  • Ability to work effectively in a team-oriented environment
  • Prior experience in a skilled nursing facility or geriatric rehabilitation is a plus
