Job Summary:

As a Physical Therapist at our Skilled Nursing Facility, you will play a crucial role in assessing and providing rehabilitation services to residents with physical limitations and functional impairments. You will work closely with a multidisciplinary team to develop and implement individualized treatment plans aimed at optimizing residents' physical function, mobility, and overall quality of life.

Key Responsibilities:

  1. Patient Assessment: Conduct thorough assessments of residents to evaluate their physical abilities, functional status, and rehabilitation needs.
  2. Treatment Planning: Develop individualized treatment plans that outline specific goals and interventions based on assessment findings.
  3. Therapeutic Interventions: Provide physical therapy interventions, including exercises, therapeutic modalities, manual techniques, and gait training, to improve residents' strength, mobility, balance, and overall function.
  4. Documentation: Maintain accurate and comprehensive electronic health records, including initial evaluations, progress notes, and discharge summaries, in compliance with facility policies and regulatory standards.
  5. Education: Educate residents and their families on the goals, benefits, and techniques of physical therapy treatments, as well as home exercise programs to support continued progress.
  6. Collaboration: Collaborate closely with the interdisciplinary healthcare team, including occupational therapists, speech-language pathologists, and nursing staff, to ensure coordinated and holistic care for residents.
  7. Adaptive Equipment: Recommend and facilitate the use of assistive devices and adaptive equipment to enhance residents' independence and safety.
  8. Safety: Adhere to infection control protocols, safety measures, and ethical standards to maintain a safe and supportive rehabilitation environment.


  • Master's or Doctorate degree in Physical Therapy from an accredited program
  • State licensure as a Physical Therapist
  • Strong interpersonal and communication skills
  • Empathy and patience when working with residents and their families
  • Ability to work effectively in a team-oriented environment
  • Prior experience in a skilled nursing facility or geriatric rehabilitation is a plus
