Job Description: . Under the general direction of the Director of Nursing, the RN Complex Care Manager will work in conjunction with the Complex Care Medical Director to provide care management to the most acute and vulnerable patients who are enrolled in the Complex Care Team.

Requirements:

•       Graduation from an accredited school of nursing, current license to practice as an RN in the Commonwealth of Massachusetts

•       1-3 years’ experience in a community health or ambulatory setting

•       Current CPR certification  

•       Demonstrated capability and confidence in performing patient assessments and triage.

•       Sensitivity/skill in working with patients with complex medical, psychosocial, and socioeconomic circumstances such as patients with significant mental illness, homelessness, poverty, and substance use disorders. 

•       Ability to function effectively both as a leader and a member of a high functioning healthcare team, as well as a proven ability to work independently.

•       Ability to work in a culturally diverse environment. 

•       Bilingual/bicultural individuals who can speak Spanish and/or Portuguese are an asset

•       Experience with and proficiency in the use of electronic health records and telehealth platforms. Experience with Epic preferred.

•       Strong computer skills required.
