Skilled Nursing & Rehabilitation

SPECIALTY UNIT: Retirement / Senior Living / Nursing Home

LOCATION: Grand Rapids, MI

SHIFT:

  • 12 Hour Shift: 7a to 7p and 7p to 7a
  • 8 Hour Shift: 3p to 1130p and 11p to 730a

Job Description:


The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized nursing care to residents. Collaborates with resident and family, other inter-disciplinary colleagues, including providers, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting. 

The Registered Nurse (RN) is to ensure safe, reliable administration of resident care and resolution of care and service concerns. Cares for residents in all phases of preventative care, health maintenance, diagnosis, rehabilitation, and treatment. Responsible for the coordination of care, resident assessment, resident education, and various other nursing interventions.


Responsibilities:


  • Responsible for resident care and ensuring residents receive care outlined in their care plan. 
  • Assess the psychological, social, spiritual, educational, developmental, and cultural needs of patients.
  • Review patient history with the patients and/or their family members and assure completion within an appropriate timeframe.
  • Review all available information obtained by other health care team members.
  • Formulate an individualized care plan using the assessment data, as well as the input of the patient, family, and healthcare team. 
  • Assess, plan, implement, and evaluate documents in the patient record.
  • Work as an advocate for the physical and emotional well-being of the patient.

Requirements:


Education, Training, and Experience:


  • Licensed to practice in the state of Michigan, Bachelor’s Degree in Nursing is preferred.
  • May be required to have professional knowledge of psychiatric nursing theory as required through completion of education from an accredited school of nursing.
  • Satisfactory previous clinical experience.
  • New grads can be considered but would prefer pysch or ED experience.

  • Analytical skills necessary in order to collect, analyze and interpret complex patient data, to determine importance of information collected and the ability provide this data to providers and other team members as necessary.
  • The candidate may be required to have a valid Michigan driver’s license and ability access company Transportation Vehicles for the performance of job duties.
