Job Description:

We are looking to hire a professional and passionate Clinical Pharmacist Specialist - Pediatric Cardiology to be responsible for providing specialized pharmaceutical care to pediatric cardiology patients in a healthcare organization. You will work collaboratively with the healthcare team to optimize medication therapy and promote safe, effective, and evidence-based medication use.

Responsibilities:

• Serve as a clinical pharmacy resource for the pediatric cardiology team, including physicians, nurses, and other healthcare providers.
• Review medication orders, assess appropriateness of therapy, and make recommendations to optimize medication regimens for individual patients.
• Provide drug information and education to healthcare providers and patients.
• Monitor patients for drug-related problems and adverse drug reactions and make recommendations for dose adjustments or alternative therapies as necessary.
• Collaborate with the healthcare team to develop and implement medication therapy management plans.
• Participate in rounds with the pediatric cardiology team to review patients' medication therapy and make recommendations.
• Assist with the management of complex medication regimens and medication-related procedures, such as anticoagulation therapy.
• Develop and implement clinical protocols and guidelines to improve medication therapy management in pediatric cardiology patients.
• Participate in research and quality improvement projects related to medication therapy management in pediatric cardiology patients.
• Provide education and training to pharmacy residents, students, and other healthcare professionals.
• Maintain current knowledge of pediatric cardiology medications, disease states, and treatment guidelines.
• Participate in professional organizations and conferences to network with peers and stay up to date on industry developments.

Requirements:

• Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
• Completion of a residency in pediatric pharmacy or cardiology pharmacy, or equivalent experience in pediatric cardiology pharmacy.
• Current state licensure as a pharmacist.
• Board certification in pharmacotherapy or specialty pharmacy (preferred).
• Minimum of 3-5 years of experience in clinical pharmacy practice, preferably in a pediatric cardiology setting.
• Strong clinical knowledge of pediatric cardiology medications, disease states, and treatment guidelines.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with healthcare providers and staff.
• Strong analytical and problem-solving skills.
• Ability to manage multiple priorities and projects simultaneously.
• Proficiency in computer systems and pharmacy software applications.
