Job Title: Nurse Practitioner - PACE Program


Overview:

We are seeking a highly skilled and compassionate Nurse Practitioner to join our innovative Program of All-Inclusive Care for the Elderly (PACE) team. As a vital member of our interdisciplinary healthcare team, the Nurse Practitioner will play a key role in delivering comprehensive and person-centered care to elderly individuals enrolled in the PACE program.


Responsibilities:

Clinical Assessment and Care Planning:

  • Conduct thorough health assessments to identify the unique needs and preferences of PACE participants.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team to address the physical, emotional, and social aspects of participants' well-being.


Primary Care Services:

  • Provide primary care services, including diagnosing and treating acute and chronic conditions.
  • Prescribe medications and order diagnostic tests as necessary, ensuring adherence to evidence-based guidelines.


Coordination of Care:

  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate and integrate care for PACE participants.
  • Facilitate communication among team members to ensure continuity and quality of care.


Patient Education:

  • Educate participants and their families on health management, preventive care, and treatment plans.
  • Promote self-care and empower participants to actively engage in their healthcare decisions.


Health Promotion and Disease Prevention:

  • Implement and oversee preventive health initiatives and screenings to maintain the overall health and well-being of PACE participants.
  • Participate in community outreach and education programs to promote healthy aging.

Documentation and Compliance:

  • Maintain accurate and up-to-date electronic health records, documenting all assessments, interventions, and communication with participants and other team members.
  • Ensure compliance with regulatory standards and PACE program requirements.


Qualifications:

  • Current and unrestricted Nurse Practitioner license in the state of practice.
  • Certification as a Family or Adult-Gerontology Nurse Practitioner.
  • Experience in geriatric or primary care, preferably within a PACE or similar program.
  • Strong clinical assessment and diagnostic skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively in a multidisciplinary team environment.
  • Knowledge of Medicare and Medicaid regulations related to PACE programs.
