Job Description:

The Primary Nurse -RN is responsible for the general oversight, assessment, planning, and construction of the nursing care delivered to the participant. The primary nurse may delegate some direct care, data collection, and nursing assessment responsibilities to other nurses and nursing assistants on the team but retains responsibility for ensuring that interventions are carried out in accordance with the participant’s care plan and the orders of the participant’s medical providers. The RN is responsible for the delivery and documentation of quality patient care in accordance with departmental policies and procedures, and in collaboration with other members of the Interdisciplinary Team. RNs may provide care in the PACE Day Health Center or in participant’s homes.


  • Coordinate with doctors and other healthcare professionals regarding their patients.
  • Coordinate patient medications between the pharmacists and doctor to ensure that patients effectively manage precise dosages.
  • Assess the patient’s needs and keep proper documentation of patient care.
  • Assess the psychological, social, spiritual, educational, developmental and cultural needs of patients.
  • Review patient history and physical with the patients and/or their family members and assure completion within an appropriate timeframe.
  • Review all available information obtained by other health care team members.
  • Formulate an individualized care plan using the assessment data, as well as the input of the patient, family, and healthcare team.
  • Evaluate patient progress towards goals and expected outcomes in collaboration with other health care team members.
  • Assess, plan, implement, and evaluate documents in the patient record.
  • Work as an advocate for the physical and emotional well-being of the patient.

  • Baccalaureate degree in nursing preferred
  • Current state license in nursing
  • Current BLS certification required



  • Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred, equivalent training or related experience considered.
  • A minimum of one year experience working with a frail or elderly population.
  • Prior experience in interdisciplinary team setting a plus.
