Care Manager - Registered Nurse (RN)

SHIFT: M-F 9-5, weekend rotation

Job Description:

We are looking to hire an empathetic care manager to create individualized care plans for patients. The care manager will perform the initial assessment on admission screening for all patients to ensure they receive the appropriate supportive services. The Care Manager will ensure all healthcare providers involved are up to date with case information and collaborate with patients' families, friends, and caretakers to effectively share information. The care manager will continually develop a network of quality providers and keep accurate patient records. You will link patients to community programs and entitlements, such as housing services and self-management programs.

To ensure success you will guide patients through the healthcare system and cultivate relationships with healthcare providers. Top candidates can multitask in a dynamic environment and have excellent negotiation skills.



Responsibilities:

  • Completes a comprehensive initial assessment and creates/updates personalized care plans for patients.
  • Performs all activities for multidisciplinary care coordination.
  • Communicate and collaborate all relevant patient information to appropriate health team members to facilitate patients' treatments.
  • Collaborate with patients' families, friends, and social supports in developing treatment plans.
  • Suggest alternative treatment plans when patients' services requests do not meet medical necessity criteria.
  • Instruct and educate patients on procedures, healthcare provider instructions, and referrals.
  • Link patients to social services programs such as transportation assistance and translation services.
  • Conduct regular follow-ups with patients to evaluate progress, promote continuity of care, and ensure improved health outcomes.
  • Understands and adheres to our hospital performance standards, policies, and behaviors.
  • Performs all other related duties as assigned.

Requirements:

  • Current RN license in the state of practice.
  • Bachelor’s degree of an accredited school of nursing required.
  • Five (5) years of clinical/hospital Critical Care or Med Surg experience required.
  • Certified Case Manager (CCM) certification preferred. 
  • Previous case management experience preferred.
  • Knowledge of healthcare financial, regulatory, and payer issues preferred.
  • Knowledge of state, local, and federal programs strongly preferred.
  • PRI/Screen certification strongly preferred.
  • Proficient with Healthcare Management Systems and Microsoft Office Word and Excel.
  • Outstanding communication skills, both written and verbal.
  • Excellent organizational and negotiation skills.
