Job Summary:
We are in search of a professional Pharmacist Clinical Coordinator who will oversee the clinical pharmacy services, ensuring optimal patient care through effective medication management and collaboration with the healthcare team. This role involves developing and implementing clinical programs, providing education, and supporting the pharmacy team in delivering evidence-based care.
Responsibilities:
- Coordinate and manage clinical pharmacy services across the healthcare facility.
- Develop and implement clinical programs to improve patient outcomes.
- Conduct medication therapy management and review for patients.
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care.
- Provide education and training to pharmacy staff and other healthcare providers.
- Monitor and evaluate clinical pharmacy activities and outcomes.
- Ensure compliance with regulatory standards and guidelines.
- Participate in quality improvement initiatives and clinical research projects.
- Serve as a resource for clinical information and support to the pharmacy team.

Requirements:
- Doctor of Pharmacy (Pharm.D.) degree from an accredited school of pharmacy.
- Valid pharmacist license in state of practice.
- Two (2) years Clinical Pharmacist Hospital experience
- Strong knowledge of pharmacotherapy and evidence-based medicine.
- Excellent communication, leadership, and interpersonal skills.
- Ability to work effectively in a multidisciplinary team environment.
- Proficiency in electronic health records (EHR) and pharmacy information systems.
