About the Role

We are seeking a dedicated Physical Therapist to join our interdisciplinary team (IDT) in providing quality patient-centered care. The Physical Therapist will play a key role in care planning and collaboration, focusing on coordinating and providing restorative and rehabilitative Physical Therapy services to participants both at our center and in their homes.

Key Responsibilities

• Conduct Physical Therapy assessments in our facility and in homes
• Participate in IDT meetings to develop participant care plans
• Deliver and document Physical Therapy interventions
• Provide various therapy treatments including exercise, muscle re-education, gait training, and ADL functional training
• Supervise Therapy Assistants and implement Functional Maintenance Programs
• Train staff, participants, and caregivers in therapeutic equipment use and care

Required Qualifications

• Bachelor's Degree in Physical Therapy
• Five years of Physical Therapy experience
• Valid Physical Therapy License in the state of practice, or proof of License eligibility

Preferred Qualifications

• Master's Degree or Doctorate in Physical Therapy
• Two years of experience with frail or elderly population
• Current CPR certification
