Job Description:
We are looking to hire a professional and caring Licensed Practical Nurse/Care Coordinator. You will work closely with interdisciplinary healthcare teams to develop and implement care plans, manage patient care transitions, manage patient records, and coordinate care plans. You will monitor patient progress, address concerns, and work to improve overall patient outcomes. The ideal candidate will be an organized multitasker who can efficiently balance patient care coordination with clerical responsibilities.
Responsibilities:
· Collaborate with healthcare providers, patients, and families to develop individualized care plans, ensuring all medical, social, and emotional needs are met.
· Serve as a liaison between patients, families, and healthcare providers, advocating for patient needs and preferences.
· Provide education to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
· Track patient progress, identify potential issues, and coordinate follow-up care to ensure continuity and quality of care.
· Maintain accurate and up-to-date patient records, care plans, and reports in compliance with healthcare regulations and standards.
· Assist with navigating the healthcare system, including arranging appointments, referrals, and transportation as needed.
· Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:
· Current LPN (Licensed Practical Nurse) license in State of practice.
· Minimum of 6 Months Long Term Care experience.
· Geriatric behavioral health experience preferred.
· Strong communication, interpersonal, and organizational skills.
· Ability to work effectively in a team-oriented environment.
· Proficiency in MS Suite, electronic medical records (EMR) and other relevant healthcare software.
· Compassionate, patient-centered approach to care.
· Basic Life Support (BLS) certification required.
