JOB TYPE: Full-Time Permanent
The renal dietitian provides expert guidance on diet and nutrition to enhance the quality of life for patients, especially those with kidney disease. This role involves conducting thorough nutritional assessments and working closely with interdisciplinary teams to develop personalized care plans. The dietitian educates patients and caregivers on renal-specific dietary needs, monitors health metrics, and adapts care strategies based on the latest research and evidence-based practices.
Responsibilities:
- Guides patients in making informed diet and nutrition choices to improve their health and well-being.
- Performs detailed nutrition assessments for new and high-risk patients.
- Collaborates with interdisciplinary teams to create individualized care plans, track progress, and update goals monthly.
- Identifies and implements evidence-based nutritional interventions to meet patient needs.
- Educates patients and their families/caregivers on managing renal diets and fluid intake.
- Monitors monthly lab results and weight changes, recommending adjustments to meet health goals.
- Uses a patient-centered approach, considering individual and cultural preferences to engage patients effectively.
- Participates in Quality Assessment Performance Improvement (QAPI) processes, including documentation and attending clinic meetings.
- Develops and distributes educational materials to support adherence to renal diet guidelines.
- Communicates nutrition-related concerns with the care team, including physicians.
- Stays informed on advancements in kidney disease and nutrition, sharing insights with the care team as necessary.
Requirements:
- Must be a Registered Dietitian Nutritionist with Commission on Dietetic Registration.
- Minimum of one year of professional experience in clinical nutrition as a registered dietitian (after successful completion of registration exam) required.
- Previous renal care experience preferred, not required.
