Job Description:
We are seeking a highly skilled and compassionate Hematopoietic Transplant Coordinator to join our team in Gainesville, FL. The coordinator will be responsible for managing all aspects of patient care related to hematopoietic stem cell transplantation, including pre-transplant evaluation, donor matching, and post-transplant follow-up. This position plays a vital role in guiding patients and their families through the complex transplant process, ensuring a smooth and safe experience.
Key Responsibilities:
- Coordinate the entire hematopoietic transplant process from patient referral, evaluation, donor matching, through to post-transplant follow-up.
- Serve as a liaison between patients, families, physicians, and the transplant team to ensure clear communication and coordination of care.
- Educate patients and their families on the transplant process, including risks, benefits, and post-transplant care expectations.
- Oversee the scheduling of diagnostic tests, transplant procedures, and follow-up appointments.
- Collaborate with a multidisciplinary team, including hematologists, oncologists, transplant physicians, and nurses, to develop individualized patient care plans.
- Coordinate donor search and selection in conjunction with national and international registries.
- Monitor patient progress before and after transplant, managing complications and adjusting care plans as needed.
- Maintain detailed and accurate records of transplant procedures, donor information, and patient outcomes in electronic health records (EHR).
- Ensure adherence to clinical protocols and regulatory guidelines related to transplant procedures.
Qualifications:
- Active Registered Nurse (RN) license in the state of Florida.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 3 years of experience in hematology, oncology, or transplant nursing.
- Experience with stem cell transplantation or cellular therapy highly preferred.
- Excellent communication, organizational, and leadership skills.
- Ability to work collaboratively in a multidisciplinary team environment.
- Proficiency in using electronic health records (EHR).
