Status: Full-Time, Day Shift (M-F 9A-5P with weekend rotation)

Salary Range: $88,000 to $130,000

Sign On Bonus: $5,000

About the Role:

As a Medical Social Worker you'll play a vital role in providing comprehensive psychosocial support to patients and their families. You'll work collaboratively with a multidisciplinary team to develop and implement individualized treatment plans, addressing the emotional, social, and practical needs of patients facing complex medical challenges.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments to identify patient needs.
• Develop and implement individualized treatment plans.
• Provide counseling, crisis intervention, and grief support.
• Coordinate with community agencies to ensure continuity of care.
• Advocate for patients' rights and interests.
• Contribute to the development and delivery of educational programs.
• Maintain accurate and detailed documentation.

Qualifications:

• Master's degree in Social Work from an accredited program.
• Licensed Social Worker (LMSW/LCSW) in New York State.
• Minimum of two years of experience in an acute care hospital setting or related field.
• Strong clinical skills and ability to work effectively in a fast-paced environment.
• Excellent communication and interpersonal skills.
