Position: Nurse Practitioner
Location: Glendale, Antelope Valley Greater Los Angeles (Downtown, East LA, South LA – including areas like Downey, Southgate, and Alhambra)
Schedule: Full-time, Monday–Friday with some flexibility
Work Environment: A mix of on-site care in dialysis and specialty clinics, in-home patient visits, and occasional virtual appointments

What You’ll Do
- Provide person-centered care to high-risk, often underserved patient populations
- Manage comprehensive health assessments and chronic condition oversight
- Create clear, actionable care plans and work closely with patients and their families to ensure understanding and follow-through
- Coordinate with nephrologists and care teams to align on care plans and improve outcomes
- Conduct visits in dialysis centers, nephrology clinics, and patient homes
- Handle clinical documentation, coding, and care coordination with accuracy
- Flex between in-person visits and telehealth as needed, based on patient accessibility
What We’re Looking For
- Active Nurse Practitioner license in California
- Board certified (AANP or ANCC)
- At least two years of experience in adult primary care, internal medicine, geriatrics, or related fields
- Skilled in chronic disease management; knowledge of HCC coding is a plus
- Current DEA license and BLS certification
- Comfortable navigating different care settings and working independently in the field
- Driver’s license and access to reliable transportation
