Job Description:

We are currently seeking a dedicated and compassionate PrEP Navigator LPN to join our team. You will play a crucial role in HIV risk assessments, promoting public health initiatives, preventing disease, and ensuring the community's overall well-being. You will work collaboratively with a team of healthcare professionals and community partners to implement and coordinate various programs and services.

Responsibilities:

• Conduct health assessments and screenings within the community to identify potential health risks and concerns.
• Develop and implement educational information to promote health and wellness.
• Provide education on risk reduction and health interventions.
• Maintain accurate and confidential patient records, ensuring compliance with healthcare regulations.
• Offer counseling and support to individuals and families regarding health-related issues and resources.
• Contribute to developing policies and guidelines based on research and best practices.
• Stay current with advancements in nursing practices through continuing education.

Requirements:

• Current LPN (Licensed Practical Nurse) license in State of practice.
• 3 years of experience
• CPR and Basic Life Support (BLS) certification required.
• HIV knowledge preferred
• Strong knowledge of public health principles, epidemiology, and preventive care.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of a team and independently.
• Proficient computer skills for record-keeping and data analysis.
• Compassionate, patient-centered approach to healthcare.
