Position: Hospice Admissions Nurse (RN)
Location: Columbus, OH
Schedule: Day Shift
Travel: Required – *350 Gas Card Provided + Auto Allowance
About the Role:
We are seeking a compassionate and skilled Hospice Admissions Nurse (RN) to join our team, serving patients and families within the Columbus, OH. metro area. In this critical role, you will be the first point of contact for patients entering hospice care, ensuring a seamless and supportive transition. You’ll provide education, conduct assessments, and collaborate with interdisciplinary teams to develop individualized care plans that honor patient wishes.
Key Responsibilities:
- Conduct in-home and facility-based admissions for new hospice patients, assessing eligibility and establishing care plans.
- Educate patients and families on hospice services, expectations, and symptom management.
- Collaborate with physicians, social workers, and care teams to ensure continuity of care.
- Complete timely and thorough documentation in accordance with Medicare/Medicaid and organizational standards.
- Provide compassionate support and guidance during a sensitive and emotional time for families.
- Travel within a one-hour radius of Columbus, OH, meeting patients in their homes, hospitals, or skilled nursing facilities.
- Maintain compliance with hospice regulations, policies, and best practices.
What We’re Looking For:
- Active RN license in Ohio (required).
- Prior hospice or palliative care experience preferred, but not required—training provided.
- Strong communication and assessment skills, with the ability to guide patients and families through the hospice decision-making process.
- Ability to work independently in the field while collaborating closely with an interdisciplinary team.
- Reliable transportation and willingness to travel within a one-hour radius (mileage reimbursement provided).
