Job Summary and Qualifications
The Facility Case Management Director is responsible for the overall management and coordination of case management operations within an acute care setting. This role ensures departmental alignment with internal policies and external regulatory standards while promoting integration across patient care, discharge planning, and other hospital functions to support continuity of care and efficient resource utilization.
This leadership role oversees all aspects of day-to-day case management, guiding interdisciplinary teams and external partners to achieve clinical and operational goals. The Director must demonstrate executive presence, strong communication and presentation skills, and a consistent record of driving results. This role requires expertise in case management, leadership, team building, and change management. The ideal candidate is a proactive problem-solver who can manage competing priorities and foster collaboration across departments.
Key Responsibilities:
- Lead and evaluate departmental operations, including staffing models, onboarding, training, and technology utilization to support performance and quality standards.
- Strategically allocate resources to meet department productivity and service goals.
- Oversee core case management functions, including care coordination and discharge planning processes, ensuring timeliness, quality, and compliance.
- Collaborate with finance and senior leadership to meet operational and strategic objectives.
- Monitor performance metrics, analyze data, and implement improvements to enhance departmental outcomes.
- Ensure compliance with internal policies, standard operating procedures, and regulatory guidelines.
- Cultivate relationships with key stakeholders, including medical, nursing, ancillary, ethics, and legal teams.
- Maintain adherence to organizational values and ethical standards.
- Perform additional responsibilities as required.
Qualifications:
- Minimum of 2 years of recent acute care case management experience.
- 5–7 years of overall clinical experience.
- Bachelor’s degree required; advanced degree in Nursing, Health Administration, or Business Administration preferred.
- Must hold current RN licensure (LCSW not accepted for this role).
