Job Title: Clinical Pharmacist

SHIFTS available: 6:00am – 4:30pm or 9:30am -8:00pm or Night shift (7 on / 7 off)
Job Type: Full-time
Position Summary:
The Clinical Pharmacist is responsible for providing comprehensive medication management and clinical pharmacy services to hospitalized patients at an acute care facility. This role works collaboratively with physicians, nurses, and other healthcare professionals to ensure the safe, effective, and cost-efficient use of medications. The Clinical Pharmacist also contributes to quality improvement initiatives, clinical protocol development, and staff education within the hospital.
Key Responsibilities:
- Evaluate and verify physician medication orders for appropriateness, accuracy, and safety.
- Provide pharmacokinetic and therapeutic drug monitoring and adjust dosing as necessary.
- Participate in multidisciplinary rounds and contribute to patient care decisions.
- Educate patients and caregivers on appropriate medication use upon discharge.
- Monitor for adverse drug reactions, drug interactions, and therapeutic duplications.
- Serve as a drug information resource for medical staff, nursing, and allied health professionals.
- Collaborate in the development and implementation of clinical pathways, order sets, and treatment protocols.
- Support transitions of care through medication reconciliation at admission, transfer, and discharge.
- Document clinical interventions and outcomes in the electronic health record.
- Precept pharmacy residents and students, as applicable.
- Participate in quality assurance, formulary management, and performance improvement activities.
Qualifications:
Required:
- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Current and active pharmacist license in Florida.
- Completion of a PGY1 Pharmacy Practice Residency or equivalent hospital experience.
- Strong clinical knowledge base and problem-solving skills.
- Excellent communication, organizational, and interpersonal skills.
- **Open to new grads, who completed rotations in an acute care setting**
Preferred:
- Experience in an acute care hospital setting.
- Familiarity with electronic health records (EHR) and pharmacy information systems.
