Nurse Practitioner (NP)

Position Overview
A full-time opportunity is available for a Board-Certified Nurse Practitioner to provide specialized care in wound and ostomy management across both inpatient and outpatient settings. The role involves pre-surgical education and site markings, post-operative care, and comprehensive patient education to support recovery and improve quality of life. The provider will work closely with surgical teams to ensure continuity and excellence in care delivery.
Key Responsibilities
- Conduct thorough assessments of patients with ostomies, including evaluation of wound status and skin integrity.
- Develop individualized care plans to support healing and manage complications.
- Educate patients and their families on ostomy care techniques, skin care maintenance, and lifestyle adjustments.
- Collaborate with multidisciplinary teams, including surgeons and nursing staff, to coordinate comprehensive care.
- Apply evidence-based practices and advanced wound care techniques to promote recovery and reduce infection risk.
- Maintain accurate and compliant documentation of assessments, interventions, and patient outcomes.
- Provide site markings and education prior to colorectal or urologic surgical procedures.
Qualifications
- Board Certification as a Nurse Practitioner (required by start date)
- Active or eligible for New York licensure or willing to obtain
- WOCN Certification (Wound, Ostomy, and Continence Nursing) – Required
- DEA license (or willingness to obtain)
- At least 1 year of NP experience, preferably in wound/ostomy care – highly preferred
- Recent or upcoming graduates (2025) may be considered
- Excellent communication skills and a patient-centered approach to care
