POSITION TYPE: Primary Care Nurse Practitioner – New Clinic!
LOCATION: Twin Falls, Idaho
SHIFT: 8am-5pm (M-F)
As a Primary Care Nurse Practitioner, you will take a lead role in delivering personalized, high-quality care to a panel of senior patients in a primary care setting. You will work collaboratively with a supportive care team, including Medical Assistants or LPNs, a Front Office Coordinator, Clinic Manager, and Medical Director. You will provide holistic, coordinated, and relationship-based care. This position offers the opportunity to make a meaningful impact through high-touch, team-oriented healthcare in a patient-centered environment.
Key Responsibilities
- Provide full-scope primary care (acute, chronic, preventive)
- Conduct Annual Wellness Visits and Health Risk Assessments
- Manage patient panels and coordinate care with support staff
- Order/interprets labs, perform screenings, and provide education
- Make referrals, complete prior authorizations, and post-discharge follow-ups
- Deliver care via in-clinic, telehealth, or occasional home visits
- Ensure accurate HCC coding and meet quality benchmarks
- Maintain timely and complete charting in the EHR
- Leads clinic operations, team meetings, and community outreach
- Mentor new providers and precept students, as needed
- Delivers high-quality patient care and fosters an exceptional patient experience, as demonstrated by strong patient satisfaction scores.
- Effectively collaborates and communicates with all members of the care team to ensure patient needs are met seamlessly during clinic visits and beyond, supporting a smooth and coordinated care journey.

Requirements
- Active FNP or AGNP license in Idaho
- DEA license and prescriptive authority
- Strong EHR and Microsoft Office skills
- CPR/BLS certification
- Proficiency in MS Word, Excel & PowerPoint
- Valid Idaho driver’s license
Preferred
- Adult-Gerontology certification
- Experience with value-based care and new practice growth
- Background in patient education and care coordination
