POSITION TYPE: Nurse Practitioner, AWV (Annual Wellness Visits) – Hybrid Role in New Clinic!
LOCATION: Twin Falls, Idaho
SHIFT: 8am-5pm (M-F)
This position is a full-time Nurse Practitioner opportunity. This role includes in clinic, virtual visits, and in-home visits. Home visits will be scheduled within a 30-mile radius of your home region (Idaho Falls, ID). This opportunity focuses on delivering comprehensive, high-touch primary care to adult and senior patients. Care is provided across clinic, virtual, and in-home settings. The NP is responsible for managing a patient panel with support from a multidisciplinary care team.
Key Responsibilities:
Provide comprehensive primary care for acute and chronic conditions, including exams, diagnoses, treatments, and preventive services, across clinic, virtual, and home settings. Collaborate with a multidisciplinary team to coordinate care, support onboarding, and contribute to care planning. Maintain high standards for documentation, quality metrics, patient satisfaction, and regulatory compliance, including accurate coding and use of the EHR.
Qualifications:
- Active Nurse Practitioner license and DEA
- Certification through ANCC or AANP
- 3–5+ years of independent experience in primary care (required)
- Experience with complex, elderly, or underserved populations preferred
- Strong communication, clinical judgment, and documentation skills
- CPR/BLS certification
- Valid driver’s license and willingness to travel locally (within 30 miles)
Preferred Experience:
- Home health or in-home care
- Value-based care models and care coordination
- New clinic or program implementation
- Adult-Gerontology certification

- Proven ability to work efficiently in a fast-paced clinical environment while continuously improving workflows
- Skilled in building trust and rapport with elderly patients using empathy and effective communication
- Highly detail-oriented and consistent in following routines and protocols
- Receptive to feedback with a commitment to improving clinical documentation and coding accuracy
- Proficient with EMR systems and knowledgeable in Medicare HCC coding and managed care models
- Flexible with scheduling and adaptable to different care settings (clinic, virtual, home visits)
- Willingness to travel locally as needed to meet patient care demands
