Clinical Team Manager (RN) – Home Health
Location: Valparaiso IN
Shift: Days Monday – Friday plus Admin On Call 1, maybe 2 times per month
We’re seeking a passionate Clinical Team Manager with a background in home health to oversee clinical operations and ensure patients receive the highest quality care. This leadership role supports both patients and care teams with strategic direction, empathy, and excellence.
What You’ll Do:
- Provide oversight for all patient care services and staff assignments
- Coordinate referrals and ensure timely initiation of care
- Review, approve, and update individualized patient care plans
- Maintain regulatory compliance and clinical standards
- Facilitate regular OASIS reviews and clinical audits
- Mentor and coach field staff, fostering team success and communication
- Coordinate physician orders and communication between stakeholders
- Travel locally as needed to support clinical teams and patient needs
What You Bring:
- Associate degree in Nursing (minimum)
- RN license (unrestricted and valid in the state of practice)
- 2+ years of RN experience in home health or hospice
- 1+ year in a supervisory/leadership role
- Proficiency with Homecare Homebase, Workday, or Microsoft Office a plus
- Ability to lift 50–100 lbs and meet physical demands of a mobile work environment
- Reliable vehicle, valid driver’s license, and auto insurance
