SHIFT: Day (8:00am-4:30pm)(M-F, No night or weekends)

Job Description:
We are seeking a licensed professional (LSW or RN) to join our team as an Assessor. In this role, you’ll focus on evaluating individual needs, determining program eligibility, and connecting clients and their families with the resources that best support their goals.
As an Assessor, you’ll conduct person-centered assessments, educate and support individuals and caregivers, and advocate for client needs while maintaining accurate documentation. This position blends meaningful client interaction with structured evaluation and requires both compassion and strong attention to detail. You’ll also collaborate with internal teams and community partners to ensure clients are guided toward the right programs and services.
Responsibilities:
- Carry out comprehensive, person-focused assessments to determine service eligibility.
- Offer guidance, education, and resources to clients, caregivers, and representatives to help them understand available options.
- Serve as an advocate for individuals, particularly in situations where safety or well-being is at risk.
- Identify and report potential abuse or neglect according to established guidelines.
- Document all assessments and interactions accurately and promptly in electronic systems.
- Collaborate with colleagues and external partners to coordinate client support.

Requirements:
- Current Ohio license as a Social Worker (LSW) or Registered Nurse (RN) in good standing.
- Minimum of one year of experience in fields such as medical social work, geriatrics, or home health.
- Familiarity with electronic record-keeping systems preferred.
- Understanding of aging-related issues and challenges faced by older adults.
- Solid computer skills, including comfort with MS Word, Excel, and online documentation platforms.
- Demonstrated respect for diverse cultural, ethnic, and spiritual backgrounds.
- Reliable high-speed internet access for remote system use.
- Valid driver’s license and dependable transportation for local travel across Butler, Clermont, Clinton, Hamilton, and Warren counties.
- Ability to attend occasional training sessions within Ohio.
