Nurse Practitioner – Full Time (On-Site)

We work directly with local dialysis centers, nephrology offices, and patients in their homes to improve access to care, reduce avoidable hospital visits, and create a better experience for those managing long-term health conditions.
What You’ll Be Doing
- Deliver comprehensive, relationship-based care to adult and senior patients in a mix of clinic settings with an occasional in home and telehealth
- Conduct health assessments, screenings, and chronic condition management visits
- Develop and implement individualized care plans that address both clinical and lifestyle needs
- Partner with nephrologists, dialysis center staff, and care coordinators to ensure continuity of care
- Order and interpret diagnostic tests and communicate findings with patients and families
- Maintain accurate, compliant documentation in the EHR
- Participate in team discussions to drive quality outcomes and enhance the patient experience

What You Bring
- Current California Nurse Practitioner license
- Board certification (AANP or ANCC) in Family, Adult, or Geriatric Medicine
- At least 2 years of clinical NP experience in primary care, internal medicine, or chronic disease management (home care experience a plus)
- Strong understanding of complex and chronic condition care, including CKD and diabetes
- Active DEA and CPR/BLS certification
- Reliable transportation and valid California driver’s license
- Experience with value-based care or risk-adjustment documentation a plus
Compensation and Benefits
- Base salary: $145,000–$160,000 annually
