Hospice RN (RN)
LOCATION: Rapid City, SD
JOB TYPE: Full-Time Permanent
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our hospice care team. In this role, you will plan, organize, and direct individualized care for patients and families, providing comfort, education, and support during end-of-life care. The ideal candidate is experienced in nursing with an emphasis on community health and patient education.
Key Responsibilities
Patient Care & Assessment
- Complete comprehensive initial and ongoing assessments of patients and families to determine hospice needs.
 - Develop, initiate, and regularly revise individualized plans of care in collaboration with the interdisciplinary team.
 - Utilize health assessment data to determine nursing diagnoses and establish goals that incorporate palliative nursing actions.
 - Provide professional nursing care using all elements of the nursing process, as defined by the state Nurse Practice Act.
 - Administer medications and treatments as prescribed by the physician and provide preventive and rehabilitative nursing procedures as appropriate.
 - Educate and counsel patients and families on symptom management, medication use, and self-care techniques.
 - Support patients with activities of daily living and facilitate comfort and independence whenever possible.
 - Serve as Case Manager for assigned patients, coordinating all aspects of care.
 
Communication & Coordination
- Communicate patient status updates and changes to the physician and interdisciplinary team.
 - Maintain accurate and timely clinical documentation.
 - Collaborate with community health professionals to ensure seamless, coordinated care.
 - Provide health counseling and emotional support to patients and families.
 - Participate in interdisciplinary group meetings to review patient plans and progress.
 
Additional Duties
- Participate in on-call rotation as needed.
 - Coordinate arrangements for equipment, supplies, and other necessary services.
 - Supervise hospice aides and delegate tasks as appropriate.
 - Maintain professional growth through continuing education and in-service participation.
 - Contribute to quality improvement initiatives and performance assessment activities.
 
Qualifications
- Graduate of an accredited school of nursing.
 - Current RN licensure in the state of practice.
 - 1–2 years of recent acute care experience and 1–2 years of home care experience required.
 - Bachelor’s degree and prior hospice experience preferred.
 - Valid driver’s license, reliable vehicle, and current auto insurance required.
 - Strong observation, communication, and problem-solving skills.
 - Physical ability to lift, position, and transfer patients, as well as manage supplies and equipment.
