Salary Range: Minimum – $31.94/hr  -Mid – $39.93/hr (10 + years of experience) – Max – $43.92/hr (15+ years of experience)

 

Shift Differentials? Evening Shift – $2.50  Weekend Shift – $2.00  These differentials stack. If they are working the evening shift over the weekend, they would earn $4.50/hr on top of their base pay rate.

 

Sign on: $10,000 Sign on bonus to be paid out in 5 installments over a 2-year commitment. $2,000 on their 1st or 2nd paycheck with us, then $2,000 on their 6-month, 12-month, 18 month, and 24 month checks for the total of the $10,000 SOB.

 

Relocation Assistance: Case by case basis

 

Fee: 20%

The RN CM Care Coordinator will facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management. The RN CM Care Coordinator will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization. The RN CM Care Coordinator will identify potential barriers to patient throughput and quality outcomes and will facilitate appropriate discharge plans.
