Position Title: Pharmacist

Job Summary
The Pharmacist provides and coordinates pharmaceutical care, ensuring safe and accurate medication preparation, dispensing, and clinical support for patients. This role monitors patient outcomes, collaborates with healthcare providers, and supports operational workflows within the pharmacy.
Key Responsibilities
Clinical & Patient Care
- Coordinate clinical pharmacy activities and support daily medication production and dispensing processes.
- Review and verify new medication orders for accuracy and completeness.
- Provide drug information, patient counseling, and ensure adherence to all applicable regulations.
- Conduct and document clinical assessments, monitoring drug therapy effectiveness and identifying potential issues.
- Evaluate lab results, clinical data, and other relevant information to guide therapy recommendations.
- Provide guidance to patients, caregivers, and healthcare professionals regarding medication use, handling, and storage.
Compounding & Operational Duties
- Prepare sterile and non-sterile compounded medications in accordance with professional standards and regulations.
- Perform and verify calculations related to medication preparation and compounding.
- Complete and maintain all required documentation associated with compounding and dispensing.
- Process and verify medications, supplies, and equipment before they are dispensed.
- Support selection and assessment of infusion devices and related equipment when applicable.
Quality & Compliance
- Document quality control measures and support adherence to accreditation and regulatory requirements.
- Follow established quality improvement and patient safety programs, including development of appropriate indicators or metrics.
- Maintain knowledge of pharmacy standards, compounding requirements, and industry best practices.
Collaboration & Communication
- Interact with clinical teams and attend patient care meetings as needed to support coordinated care.
- Communicate effectively with providers, staff, patients, and external partners.
- Work with other departments to support smooth operations and continuity of care.
Professional Development & Self-Management
- Maintain clinical skills through ongoing education, literature review, and participation in relevant training or professional activities.
- Work independently with minimal supervision and demonstrate strong organizational and time-management skills.
- Maintain flexibility for occasional on-call responsibilities as scheduled.
Minimum Qualifications
Education & Licensure
- Degree in Pharmacy (Bachelor’s degree required; Doctor of Pharmacy preferred).
- Active, unrestricted pharmacist license.
- Training and experience in sterile compounding and related practice standards.
Experience & Skills
- Experience evaluating and monitoring medication therapy in a clinical or home-based setting.
- Strong communication skills, both written and verbal.
- Effective organizational skills and ability to manage multiple tasks.
