About the Role:
We’re seeking a dedicated Nurse Practitioner to join our Victorville care team and is passionate about caring for medically complex populations. This role blends clinic-based care, Telemedicine with occasional in-home visits approximately a 30 mile radius around the Victorville area, allowing you to build meaningful relationships with patients while helping prevent unnecessary emergency department visits and hospital admissions.
You will work closely with an interdisciplinary care team to address medical, functional, and psychosocial needs, particularly for patients with high-risk or chronic conditions, using a holistic and preventative approach.
What You’ll Do:
- Provide longitudinal care for patients with multiple chronic and complex conditions, including cardiometabolic disease, renal disease, and other high-acuity needs
- Conduct in-depth initial and follow-up visits to evaluate symptoms, interpret labs and vitals, and thoughtfully refine care plans over time
- Produce clear, compliant, and comprehensive clinical documentation that accurately reflects patient complexity and supports risk adjustment
- Partner with patients through education focused on prevention, lifestyle choices, and ongoing condition management
- Serve as a consistent point of support for medically and socially complex patients, guiding them through a challenging and often fragmented healthcare system
- Function with a high degree of clinical autonomy while collaborating closely with physicians, care coordinators, and interdisciplinary teammates
- Facilitate seamless communication and coordination among patients, caregivers, specialists, and the broader care team
- Order and perform appropriate clinical tests, screenings, and evaluations as part of comprehensive care delivery
Qualifications:
- Active California Nurse Practitioner license
- Board-certified Nurse Practitioner in good standing (ANCC or AANP)
- Minimum of 2 years of NP experience in Primary Care, Family Medicine, Internal Medicine, Geriatrics, or Palliative Care
- Experience in home-based care is a plus
- Working knowledge of chronic care management and pharmacologic treatment options
- Familiarity with HCCs and risk adjustment documentation
- DEA licensure / prescriptive authority (required post-hire)
- Current CPR/BLS certification
- Experience working with dual-eligible, Medi-Medi, or underserved populations strongly preferred
- Bilingual English/Spanish preferred
