Hospice Community Liaison Skilled Nursing Facility Focus

Compensation: $80,000-$150,000 +
Position Summary:
We are seeking a high-performing Hospice Community Liaison with a strong Skilled Nursing Facility (SNF) focus to drive census growth and build strategic facility partnerships. This individual will serve as the primary relationship owner for assigned SNFs and be responsible for consistent referral development and education.
This is a field-based, relationship-driven role ideal for someone who thrives in post-acute healthcare sales and understands hospice eligibility and timing.
Key Responsibilities:
- Develop and maintain strong referral relationships within SNFs
- Conduct weekly facility rounding
- Provide in-services and hospice education to SNF staff
- Collaborate with DONs, administrators, social workers, and case managers
- Identify hospice-appropriate patients and facilitate smooth transitions
- Track referral activity and meet census growth goals
- Participate in care coordination meetings when appropriate
Qualifications:
- 3+ years healthcare liaison, hospice, home health, or post-acute sales experience
- Strong SNF relationship background REQUIRED
- Proven track record of census or referral growth
- Knowledge of hospice eligibility and Medicare benefit
- Excellent communication and presentation skills
- Self-motivated and territory-driven
Preferred:
- Existing SNF book of business
- Hospice-specific sales experience
- Experience presenting to clinical teams
Compensation:

Required Experience:
3–7+ years healthcare liaison, hospice, home health, or post-acute sales experience
Proven success generating SNF referral volume
Existing book of SNF relationships strongly preferred
Experience working directly with:
- SNF Administrators
- DONs & ADONs
- Case Managers / Social Workers
- Discharge Planners
- MDS Coordinators
Prior hospice experience preferred (not always required if SNF-heavy background is strong)
