Licensed Practical Nurse (LPN) – Hospice
LOCATION: Rapid City, SD
JOB TYPE: Full-Time Permanent
We are seeking a compassionate Licensed Practical Nurse (LPN) to provide hands-on patient care under the guidance of a Registered Nurse. In this role, you will support patients and families through individualized care plans, focusing on comfort, dignity, and quality of life.
This position involves working closely with an interdisciplinary care team to ensure patients receive coordinated and supportive end-of-life care. The ideal candidate is organized, empathetic, and committed to delivering thoughtful, patient-centered nursing care.
Key Responsibilities
- Deliver direct nursing care in accordance with the established care plan and nursing standards.
- Support comfort-focused care, including pain and symptom management.
- Document visits and care provided in an accurate and timely manner.
- Educate patients and caregivers on care needs and expectations.
- Observe patient conditions and report updates to the supervising nurse.
- Collaborate with interdisciplinary team members to support patient needs.
- Assist with coordinating supportive services as needed.
- Maintain proper use and care of medical equipment and supplies.
- Follow infection control and safety practices.
- Participate in ongoing training and quality improvement activities.
Qualifications
- Graduate of an accredited Practical or Vocational Nursing program.
- Active LPN/LVN license in the practicing state.
- Minimum of three years of nursing experience preferred.
- Hospice, home health, or medical-surgical experience is a plus.
- Strong communication and organizational skills.
- Valid driver’s license and reliable transportation.
- Understanding of hospice philosophy and comfort-focused care.
- Ability to support patients and families with compassion and professionalism.
