Director of Case Management (RN)
Fort Worth, Texas
Monday–Friday | Day Shift (7–10 hour days)
We are seeking an experienced Director of Case Management (RN) to lead and oversee case management operations at a well-established acute care hospital. This is a high-impact leadership role with strong executive visibility and the opportunity to drive patient outcomes and operational excellence.

👩⚕️ Key Responsibilities:
- Oversee all case management operations, including care coordination and discharge planning
- Lead, mentor, and develop a team of 10 FTEs + 7 PRN staff
- Drive performance, quality outcomes, and regulatory compliance
- Collaborate with interdisciplinary leadership to optimize patient care and throughput
- Implement process improvements and ensure efficient resource utilization
📌 Qualifications:
- Bachelor’s degree required; Master’s preferred (Nursing, Healthcare Administration, or Business)
- Active RN license required (LCSW/LMSW also considered, RN strongly preferred)
- 3+ years acute care hospital case management experience
- 2+ years leadership experience in case management
