Registered Nurse Case Manager (Hospice)
Location: Southwestern Virginia
Schedule: 40 hours/week – Monday – Friday On Call
Status: Full-time

What You’ll Do
- Lead the Team: Act as the clinical lead for the interdisciplinary team, coordinating closely with physicians and families.
- Holistic Care: Develop, implement, and document personalized patient care plans.
- Advocacy: Ensure continuity of care and monitor cases for compliance and quality.
- Mentorship: Supervise hospice aides and LPNs to maintain high standards of patient service.
- Response: Participate in on-call responsibilities to ensure our families are never alone.
Qualifications
- Education: Graduate of an accredited School of Nursing (Diploma, Associate, or BSN).
- Licensure: Current State RN License and CPR certification.
- Experience: 2+ years of clinical RN experience preferred (Home Health or Hospice experience is a plus).
- Mobility: Must have a valid driver’s license, reliable transportation, and auto insurance.
- Skills: Strong communication, problem-solving, and the physical ability to lift/transfer patients and equipment.
