Physical Therapist (PT)
Location: Alameda County + portions of Santa Clara County (including the San Jose region)
Position Type: Full-time, permanent
Work Setting: Home-based patient care / travel within assigned territory

We are seeking a motivated and compassionate Physical Therapist to provide high-quality home health services throughout Alameda County and selected areas of Santa Clara County.
In this role, you will deliver individualized care to patients in their homes while collaborating with an interdisciplinary team to support recovery, mobility, and overall well-being.
Position Details
- Full-time opportunity
- Home-based patient care throughout assigned service areas
- Typical caseload: 25–30 patient visits weekly
- Variety of visit types including:
- Start of Care (SOC)
- Initial evaluations
- Reassessments
- Discharge visits
- Routine follow-up care
- Additional clinically appropriate visits as needed
Responsibilities
As a Home Health Physical Therapist, you will:
- Complete comprehensive patient assessments and required documentation, including OASIS assessments when applicable.
- Develop and update individualized treatment plans in collaboration with physicians and care team members.
- Deliver skilled physical therapy interventions designed to maximize patient outcomes and independence.
- Assess equipment needs and provide recommendations when appropriate.
- Monitor patient progress and adjust treatment plans as needed.
- Educate patients, caregivers, and family members on treatment goals and care plans.
- Maintain timely and accurate clinical documentation and progress notes.
- Participate in care conferences, staff meetings, continuing education, and agency training programs.
- Coordinate care with interdisciplinary team members.
- Supervise Therapy Assistants and Home Health Aides when appropriate.
- Assist with discharge planning and transitions of care.
- Communicate changes in patient condition or additional care needs to leadership and physicians.
- Maintain compliance with Medicare regulations, agency standards, and infection-control guidelines.
- Demonstrate professionalism, accountability, and exceptional patient service in every interaction.
Qualifications
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in the state of employment
- Minimum one year of relevant clinical experience required
- Home health experience preferred
- Current CPR/BLS certification preferred
- Valid driver’s license with reliable, insured transportation
