Physical Therapy Assistant (PTA) – Home Health

Position Summary
We are seeking a compassionate and motivated Physical Therapy Assistant (PTA) to provide high-quality therapy services to patients in their homes. This role focuses on helping patients improve mobility, function, independence, and overall quality of life.
Full-Time, Part-Time, and PRN opportunities available.
Responsibilities
- Deliver physical therapy treatments as directed by the supervising Physical Therapist.
- Implement individualized treatment plans and home exercise programs.
- Monitor patient progress and communicate changes in condition to the Physical Therapist and care team.
- Educate patients and caregivers on exercises, mobility techniques, safety, and fall prevention.
- Maintain accurate and timely clinical documentation.
- Collaborate with physicians, therapists, nurses, caregivers, and other healthcare professionals to support optimal patient outcomes.
- Identify safety concerns within the home environment and recommend appropriate interventions.
- Participate in care coordination and interdisciplinary team meetings as needed.
- Maintain compliance with all applicable regulations and professional standards.
Qualifications
Required
- Current Physical Therapy Assistant (PTA) license in the state of practice.
- Graduate of an accredited Physical Therapy Assistant program.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and current auto insurance.
- Strong communication and organizational skills.
Preferred
- One year of clinical experience as a PTA.
- Home health experience.
- Experience working with adult and geriatric populations.
- Bilingual abilities.

Physical Requirements
