Hospice Community Liaison
Location: San Antonio, Texas
Territory: Corpus Christi and surrounding service area
Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.
Compensation: $65,000–$80,000 base salary plus uncapped commission
Position Overview
An established, patient-centered hospice provider is seeking a self-driven and compassionate Community Liaison to support continued growth throughout the Corpus Christi area.
The Community Liaison will serve as a trusted representative within the local healthcare community, developing productive relationships with physicians, hospitals, skilled-nursing facilities, social workers, discharge planners, community agencies, and other referral partners. This position combines consultative healthcare sales with meaningful community education, helping patients and families gain timely access to compassionate end-of-life care.
The successful candidate will be comfortable owning a territory, developing new referral relationships, strengthening existing accounts, and converting community engagement into measurable census growth. Key Responsibilities
- Develop and maintain trusted relationships with physicians, healthcare professionals, hospital discharge planners, skilled-nursing facilities, social workers, and community organizations.
- Execute consistent, intentional field outreach throughout the assigned Corpus Christi territory.
- Identify and develop new referral sources while strengthening engagement with existing accounts.
- Educate healthcare partners and community stakeholders about hospice eligibility, services, philosophy, and available patient support.
- Position the organization as a preferred hospice provider across the local service area.
- Develop territory strategies designed to increase qualified referrals, admissions, and overall patient census.
- Track account activity, referral trends, opportunities, and competitive developments throughout the market.
- Conduct initial conversations with patients and families regarding hospice care with empathy, clarity, and professionalism.
- Help patients and families understand available services and facilitate appropriate next steps in the admission process.
- Collaborate with clinical, admissions, and operational teams to ensure responsive service and a positive referral experience.
- Represent the organization at professional meetings, community events, networking functions, and educational presentations.
- Maintain a strong working knowledge of hospice services, referral requirements, applicable regulations, and market conditions.
Required Qualifications
- At least two years of healthcare sales, marketing, referral development, provider relations, or business-development experience.
- Demonstrated ability to generate new business and maintain productive relationships with healthcare accounts.
- Experience developing relationships with physicians, hospitals, post-acute providers, community agencies, or other healthcare referral sources.
- Strong consultative sales, communication, presentation, and relationship-management skills.
- Ability to independently manage a field-based territory and maintain consistent account activity.
- Valid driver’s license, reliable transportation, and current automobile insurance.
- Willingness to travel regularly throughout the assigned Corpus Christi service area.
Preferred Qualifications
- Hospice or palliative-care sales and marketing experience.
- Home-health, post-acute, senior-care, or closely related healthcare business-development experience.
- Established professional relationships within the Corpus Christi healthcare community.
- Bachelor’s degree in business, marketing, healthcare administration, communications, or a related field.
- Previous clinical or patient-care experience.
Additional relevant professional experience may be considered in place of the preferred educational requirement.
