We are seeking a motivated, compassionate Community Liaison to join our growing hospice team in Jacksonville, FL. This individual will be responsible for developing and maintaining strong referral relationships throughout the healthcare community while educating providers, patients, and families about hospice services. The ideal candidate is relationship-driven, self-motivated, and passionate about connecting patients with quality end-of-life care.

Responsibilities
- Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, home health agencies, and other referral sources.
- Conduct routine sales calls and educational visits to increase awareness of hospice services.
- Generate new referral opportunities while maintaining existing professional relationships.
- Educate healthcare professionals, patients, and families on hospice eligibility, services, and benefits.
- Collaborate with clinical and operational teams to ensure a seamless referral and admission process.
- Participate in community networking events, health fairs, and educational presentations.
- Maintain accurate records of referral activity and territory performance.
- Consistently meet or exceed established census growth and referral goals.
Qualifications
- Bachelor’s degree preferred; equivalent work experience may be considered.
- Minimum of two (2) years of healthcare sales or marketing experience required.
- Hospice or palliative care marketing experience strongly preferred.
- Proven ability to build and maintain relationships with physicians, healthcare providers, and community partners.
- Excellent communication, presentation, and organizational skills.
- Ability to work independently while managing a designated territory.
- Prior clinical experience is a plus.
- Valid driver’s license, reliable transportation, and current automobile insurance required.
- Ability to successfully complete all required background screenings and pre-employment requirements.
Compensation
$5k Sign-on Bonus. $75,000 – $88,000 per year (Average Base Pay Range) plus uncapped commission! Compensation is based on experience, qualifications, and market conditions
