APRN / Nurse Practitioner – Geriatric & Skilled Nursing

We are partnering with an established healthcare organization seeking an experienced APRN / NP to provide comprehensive care to geriatric patients in skilled nursing and post-acute settings.
Position Highlights
- Permanent, Full-Time Opportunity
- Monday–Friday | Day Shift
- Geriatric / Skilled Nursing patient population
- Provide comprehensive medical management for residents
- Evaluate new admissions and manage ongoing patient care
- Diagnose and treat acute and chronic conditions
- Medication management and treatment-plan development
- Collaborate closely with physicians, nursing staff, therapists and other healthcare professionals
- Strong clinical and administrative support

Ideal Candidate
- Current APRN / NP license in the state of practice
- Board Certification as an NP
- Active DEA registration, as applicable
- Geriatric, Skilled Nursing, Long-Term Care, Post-Acute, Internal Medicine, Family Medicine or related experience preferred
- Strong assessment, communication and clinical decision-making skills
- Comfortable managing medically complex geriatric patients
- Passion for providing quality, patient-centered care
