Position Type: Full-Time
Schedule: Friday, Saturday & Sunday (3×12 hr) or Friday, Saturday, Sunday & Monday (4×8 hr)
Location: Wayne, Oakland, Macomb, Washtenaw county coverage
Position Overview
We are seeking an experienced Registered Nurse (RN) to provide skilled home health services to patients in their homes and in facilities. This position is ideal for an RN with strong skilled home health experience who is comfortable working independently and managing a weekend caseload.
Requirements
- Active RN license in the state of practice
- Required: Previous skilled home health experience
- Strong clinical assessment and critical-thinking skills
- Ability to work independently and manage a patient caseload
- Excellent communication and organizational skills
- Reliable transportation and valid driver’s license
Responsibilities
- Provide skilled nursing care to patients in their homes
- Complete comprehensive assessments and develop individualized care plans
- Perform medication management, wound care, and other skilled nursing services
- Monitor patient progress and communicate changes to physicians and the care team
- Educate patients and caregivers on disease management and care plans
- Complete timely and accurate clinical documentation
- Coordinate care with the interdisciplinary team
