Clinical Pharmacist
Patients leave safer when every medication decision is precise, timely, and owned. This role puts a clinical pharmacist at the center of care—reviewing therapy in real time, closing gaps before they become harm, and giving physicians and nurses a partner they can trust on the floor. Success is measured in better regimens, fewer preventable events, and patients who understand how to take their medicines after they go home.
Performance Objectives
- Review and verify medication orders each shift so high-risk therapies are appropriate, dosed correctly, and started without avoidable delay.
- Resolve drug-related problems—interactions, allergies, duplications, and organ-function mismatches—before the first dose whenever possible.
- Lead antimicrobial and high-alert medication stewardship so targeted therapy replaces unnecessary broad coverage within agreed timeframes.
- Join interdisciplinary rounds and leave each case with a clear, accepted plan for indication, dose, duration, and monitoring.
- Counsel patients and caregivers at transitions of care so discharge regimens are understood, affordable, and followed.
- Document interventions and safety findings so trends are visible and repeat errors decline quarter over quarter.
- Support formulary and protocol use so evidence-based standards are applied consistently, not reinvented at the bedside.
Environment & Resources
You practice inside a hospital pharmacy and clinical team, partnering daily with physicians, nurses, and pharmacy technicians. Work is supported by the electronic health record, clinical decision support, and established dosing and stewardship protocols. You report to pharmacy leadership and have authority to intervene on unsafe orders within scope and policy. Coverage models, on-call expectations, and unit assignment are set with the site so priorities stay clear.
Essential Qualifications
- Active pharmacist license in the state of practice, in good standing.
- PharmD or equivalent, with hospital or acute-care clinical experience.
- Demonstrated skill reviewing complex orders—renal and hepatic dosing, anticoagulation, antimicrobials, and high-alert drugs.
- Comfort presenting recommendations on rounds and securing agreement without slowing care.
- Working knowledge of EHR order verification, intervention documentation, and medication safety reporting.
- Residency, board certification, or equivalent acute-care depth preferred for higher-acuity assignments.
