Director of Case Management
Position Type: Perm
Location: Lake Monroe, Florida | Relocation Offered
Shift Details: Days (7-10 hours/weekday only – may include occasional on-call rotations or extended days)
Key Qualifications:
- 2+ years of RECENT (within the last year) acute care Case Management leadership experience required.
- Bachelor’s Degree required.
- Master’s degree in Nursing, Health Administration, or Business Administration preferred.
- (RN) Registered Nurse or (LCSW) Clinical Social Worker license required ((RN) Registered Nurse preferred).
Key Responsibilities
- Direct and evaluate departmental operations, including the case management model, staffing (skill mix and FTEs), utilization of information technologies, onboarding, and staff competencies to achieve performance and quality objectives.
- Allocate resources effectively to meet departmental productivity and quality goals.
- Oversee core case management functions, practices, care coordination, and discharge planning processes to ensure timeliness, quality, and proper documentation.
- Partner with the Facility CFO and Division CM Leader to achieve organizational goals and expectations.
- Assess and improve department performance by evaluating operational processes, analyzing data, and implementing sustainable performance improvement activities.
- Establish and maintain strong working relationships with key stakeholders, including the CMO, CNO, ancillary service leaders, Ethics & Compliance, and Legal.
Team & Reporting Structure
- Reports To: Division VP of Case Management
- Team Oversight: 13–15 FTEs, including a Manager and Lead to support the Director
