Position Overview
As a Community Liaison, you will be the face of our hospice organization, championing our mission and promoting our specialized end-of-life care services throughout the community. Your primary focus will be building meaningful, trust-based relationships with referral sources, physicians, healthcare systems, and community partners—helping families seamlessly access compassionate care when they need it most.
In this key growth role, you will drive census expansion by increasing awareness and advocacy for our services while serving as an empathetic, knowledgeable resource for patients, families, and healthcare professionals.

Key Responsibilities
- Relationship & Account Management: Establish, nurture, and maintain strong strategic relationships with key healthcare decision-makers, including physicians, hospital discharge planners, skilled nursing facilities, assisted living communities, social workers, and case managers.
- Census & Referral Growth: Drive sustainable referral growth and achieve organizational census targets through regular, targeted outreach visits and proactive market engagement.
- Community Advocacy: Serve as an authoritative and compassionate representative of our hospice services, conducting regular educational outreach to position our organization as the provider of choice.
- Patient & Family Guidance: Support patients and families during critical decision-making moments by conducting initial consultations, clearly explaining our care philosophy, answering questions, and facilitating admissions with care and respect.
- Cross-Functional Collaboration: Partner with internal clinical and intake teams to ensure smooth transitions of care and deliver a seamless experience for referring partners and incoming patients.
Qualifications & Skills
- Education: Bachelor’s degree in Business, Marketing, Healthcare Administration, or a related field preferred (relevant experience may be substituted).
- Experience: Minimum two (2) years of healthcare sales, business development, or marketing experience required. Hospice or palliative care marketing experience strongly preferred.
- Relationship Building: Proven track record of cultivating and retaining relationships with healthcare professionals, community agencies, and clinical staff.
- Clinical Background: Prior clinical experience (e.g., RN, LPN, MSW) is a plus, though not required.
- Mobility Requirements: Must possess a valid driver’s license, reliable transportation, and active automobile insurance for regular territory travel.
