SHIFT: Evenings (3pm-11pm)

JOB TYPE: Full-Time Permanent

Job Description:

The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized nursing care to residents. Collaborates with resident and family, other inter-disciplinary colleagues, including providers, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting.

The Registered Nurse (RN) is to ensure safe, reliable administration of resident care and resolution of care and service concerns. Cares for residents in all phases of preventative care, health maintenance, diagnosis, rehabilitation, and treatment. Responsible for the coordination of care, resident assessment, resident education, and various other nursing interventions.

 

Responsibilities:

·      Responsible for resident care and ensuring residents receive care outlined in their care plan. 

·      Assess the psychological, social, spiritual, educational, developmental, and cultural needs of patients.

·      Review patient history with the patients and/or their family members and assure completion within an appropriate timeframe.

·      Review all available information obtained by other health care team members.

·      Formulate an individualized care plan using the assessment data, as well as the input of the patient, family, and healthcare team.

·      Assess, plan, implement, and evaluate documents in the patient record.

·      Work as an advocate for the physical and emotional well-being of the patient.

Requirements:

·      Minimum of 1 year of recent experience as an RN in a skilled nursing care setting.

·      Active RN license within the State of practice.

·      Graduate of an accredited school of nursing – ADN/BSN.

·      BLS & ACLS – Health Care provider

·      Exceptional understanding of health information structures and technology.

·      Exhibit brilliant empathy and communication skills.
