JOB DESCRIPTION

The Kidney Options Educator collaborates with internal and external customers to provide patients and their caregivers with renal options education and ensures a positive customer experience for every patient.


About the Role

The Kidney Options Educator collaborates with internal and external customers to provide patients and their caregivers with renal options education and ensures a positive customer experience for every patient. This will occur from first point of referral, through modality options education, access placement, follow-up and (as applicable) to eventual admission into one our dialysis centers.


The Kidney Options Educator actively collaborates with Operations and field staff and promotes home modalities to physicians, patients, and other internal/external groups utilizing the Hospital Education Program and supporting materials.


Essential Functions:

  • Establishes programs and implements renal options education for a designated area to ensure every patient/ caregiver and support system (if applicable) receives comprehensive education on all renal modality options. This includes hemodialysis treatments in the home and in-center, peritoneal dialysis, kidney transplantation, and no therapy, as well as education on dialysis access types.

  • Maintains monthly 70% patient options education and 30% community outreach, PCP and hospital relationship ratios.

  • Collaborates in the creation and implementation of patient renal options education materials, tools, and programs; including direct communication and relationship development with new or existing staff.

  • Ensures dialysis modality options are adequately addressed; programs will include orientation and support for new employees, physicians and clinical staff, as needed.

  • Has working knowledge of clinical applications, such as Patient 360, utilizes applications, and is able to provide reports guidance or support to physicians and interdisciplinary team members.

  • Maintains (minimum) 25 patents per month, options education sessions. Meets monthly patient referral and education session goals set forth in annual AIP Goals.

  • Reports monthly KPIs and updates dashboard on a regular basis.

  • Maintains open lines of communication with multidisciplinary team and respond to quality issues.

  • Establishes processes with physician practices as well as local clinics for patient referrals and evaluates the effectiveness of processes through an ongoing basis.

  • Provides patients with information and support for developing their modality option path by, tracking patient care progress, and initiating conversations with patient about potential changes.

  • Serves as the primary Patient Advocate for patient and family by promoting a patient- and family-centered care environment for optimal decision-making along the healthcare continuum.

  • Collaborates in meeting our hospital growth goals by maintaining strong physician and partner relationships. Responsibilities include but are not limited to demonstrating market awareness, identifying expansion of service opportunities, providing insight to customer relationships; both physicians and patients.

  • Maintains knowledge of Home Therapies products, services and strategies in order to promote our hospital Home Therapies to the community through outreach programs.

  • Participates in all pertinent company training and development opportunities to grow and develop business knowledge, skills and leadership competencies.

  • Provides educational programs to physicians as needed.

  • Other duties as assigned.


Minimum Education & Experience:

  • Register Nurse, Registered Dietitian or Social worker (LCSW is required for Social Worker) with minimum of 1 year of home dialysis experience required

  • Minimum of (2) years Nephrology experience; a combination of in-center, acute, peritoneal and home hemodialysis highly preferred

  • Current licensure or accreditation in the practicing state

  • Multilingual candidates preferred

Any combination of education and experience that would likely provide the required knowledge, skills, and abilities, as well as possession of any required licenses or certifications is qualifying.

  • Ability to handle confidential information discreetly

  • Ability to handle potentially sensitive situations by applying consultative skills in working with internal and external constituent groups

  • Ability to be highly organized and accountable

  • Ability to establish and maintain effective relationships with widely diverse groups, including individuals at all levels both within and outside the organization and gain their cooperation

  • Ability to work flexible hours

  • Must be able to independently travel to assigned locations

  • Able to read, write, speak, understand and satisfactorily communicate with others in English in person, over the phone and via email
